Do I Qualify?
Residency:
Potential clients must be a resident of either Columbia or Montour Counties, PA (or in areas adjacent to the county borders like Riverside and East Berwick).
Household Income Level:
Household Income must be at or below $40,000/year (You will be asked to verify your Adjusted Gross Income level for your entire Household when you submit your application).
NOTE: For residents of Snyder, Union, or Northumberland Counties who need to spay/neuter and vaccinate their pet dogs and cats CONTACT: https://www.SUNPETS.org for financial assistance and appointments.

THE APPLICATION PROCESS:
• Print, complete and mail in this PA PETS “Pets Program” Application Form HERE
• BE SURE to provide BOTH your cell and your home phone numbers, address, EMAIL ADDRESS, and the details requested on the application form for each animal.
• If you need to spay/neuter and vaccinate more than one pet dog or cat, print and complete a separate application form for each animal since we provide the veterinary clinics with specific information for each animal.
• Enclose your required copayment per animal with your application form. If you are applying for multiple animals, you can mail multiple application forms and copayments in the same envelope.
• BE SURE to sign the application form agreeing to our PA PETS DISCLAIMER to be approved for the PA PETS “Pets” Program. We will not process or approve any Application Form if it is incomplete or if the PA PETS DISCLAIMER is not SIGNED.
• Mail the application form(s) with your co-payment(s) to PA PETS, 203 East 5th Street, Bloomsburg, PA 17815. Applications and co-payments can also be dropped off at the PA PETS Thrift Store at this same address. If the store is not open, use the mail slot in the front door to drop off applications and copayments.
• You will be contacted by a PA PETS Volunteer once your application and copayment are received and approved.
COPAYMENT AMOUNTS:
Copayments are based on your Total Household’s (Annual) Adjusted Gross Income (found on page 1 of your Income Tax Return). Cash, checks or money orders are required for copayments.
• $ 5/animal
For household incomes up to $20,000/year
• $10/animal
For household incomes of $20,000 – $30,000/year
• $15/animal
For household incomes of $30,000 – $40,000/year
SCHEDULING YOUR APPOINTMENTS:
- When your completed application form(s) and your copayment(s) are approved, we will contact you by telephone.
- Our participating veterinary clinics have different price structures which we will discuss with you to answer any questions you may have, and to verify which clinic you would like to utilize for your pet.
- Our Program Manager will contact the designated veterinary clinic with your PA PETS Approval and contact information.
- THEN you can call and make your pet’s spay/neuter and vaccination appointment with the designated clinic.
- DO NOT contact any veterinary clinics prior to your call from our PA PETS with your PA PETS Application Approval.
PAYMENT OF VET CLINIC CHARGES:
The PA PETS Low-Cost Spay/Neuter Program for Pet Dogs and Cats (Our “Pets” Program) currently pays the following amounts directly to our participating veterinary clinics for our APPROVED “Pets” Program clients:
• $125 – for CAT Spay or Neuter surgeries (same amount for male or female cats)
• $150 – for MALE DOG Neuter surgeries
• $200 – for FEMALE DOG Spay surgeries
• NOTE: Most veterinary clinics have a slightly higher charge for larger dogs, based on the weight of the dogs and the amount / cost of medications required for anesthesia.
• ALL OTHER VETERINARY CLINIC CHARGES BEYOND THE STATED PA PETS PAYMENTS (above) are the responsibility of the pet owners (Pets Program Clients).
